Who Is Eligible?

You are eligible to join the HealthMarkets Care Assured^SM Plan if you are:

• Entitled to Medicare Part A; and
• Enrolled in Medicare Part B; and
• Live in the plan’s service area. (Enter your zip code in the red box to see if Care Assured is available in your town.); and
• Do not have end stage renal disease.*

Medicare-eligible beneficiaries with limited income and resources may qualify for special assistance. People under age 65 with certain disabilities may also be eligible to join Care Assured. Call your local Social Security Office for more information.

* If you developed end stage renal disease while you were a member of a health plan offered by The Chesapeake Life Insurance Company and have been continuously enrolled in a Chesapeake plan you may be eligible to enroll in Care Assured.
