What Happens Next?

Once HealthMarkets Care AssuredSM receives your completed enrollment application, you will get a confirmation letter. Care Assured will send your information to the Centers for Medicare and Medicaid Services (CMS) to verify your eligibility.

Care Assured will call you to discuss any questions you have about the Plan. If we cannot reach you by phone, you will get a Plan education letter in the mail. You will receive your Care Assured Identification Card in the mail soon thereafter.

If CMS approves your eligibility and effective date, you will receive another letter from Care Assured confirming your enrollment. Do not cancel any other Medicare Plans you may have until you get this written confirmation from Care Assured.

If, for some reason, CMS cannot verify your eligibility, Care Assured will contact you directly.

Be sure to let your providers know you are a member of Care Assured each time you seek medical care. If your providers are not familiar with Care Assured, call us so we can send them an information packet.

Check all of your medications to see if they are on the Formulary or if they require special handling such as prior authorization, quantity limits, or step therapy.

If any of the above situations apply to you, you can obtain a transitional supply of the drug upon your next refill. However, your doctor must follow the instructions indicated in the Formulary before any future prescriptions are filled.
