Enrolling online is easy and safe. Be sure to complete all sections so we can process your enrollment as soon as possible.

Automatic Monthly Payment

For added convenience, you can have your monthly premium deducted directly from your savings or checking account. Debits will be drawn on or around the 5th day of the month for which the premium is due.

The authorization will remain in effect until you notify Care Assured at least 30 days in advance of the next scheduled withdrawal. If you change banks or bank accounts, just call us and we will send you a new form.

Be sure to continue to pay the premium by mail until you receive written notification that your automatic payment has been set up.
