Rights and Responsibilities upon Disenrollment

If a member disenrolls, he/she should remember the following during the disenrollment process:

• Value Plus and Premier Plus members should use their HealthMarkets prescription drug coverage and our network pharmacies to fill their prescriptions until their coverage ends.
• If a Member leaves HealthMarkets Care Assured^SM, he or she may join another Medicare PDP or a Medicare Advantage plan as long as:
1. This type of plan is available in his area,
2. the providers are accepting new members, and
3. he/she meets the eligibility requirements of the plan.
• Members may only disenroll or switch plans during certain periods.

HealthMarkets Care Assured can disenroll a member in the following circumstances:

• Member is no longer eligible for Medicare prescription drug coverage.
• HealthMarkets is no longer contracting with Medicare or leaves the Member’ service area.
• Member moves out of HealthMarkets’s Care Assured service area.
• Member materially misrepresents a third party reimbursement.
• Member fails to pay premium.
• Member engages in disruptive behavior, provided fraudulent information when enrolling, or abused his enrollment card.
