Medical Records Management

An organized record of all aspects of your medical history is an important health management tool. Consider keeping a binder with the following information:

Health Snapshot

• List of known allergies to drugs, foods, or insects.
• List of diagnosed chronic conditions such as asthma, diabetes, high blood pressure, or high cholesterol.
• Critical information that should be disclosed in an emergency such as vision or hearing problems, epilepsy or heart trouble.

Provider Contact Information

• Names and phone numbers of all the doctors and other caregivers you see and for which condition.

Current Medications

• List of all medications you take on a regular basis for example, prescriptions, over-the-counter herbal supplements, and vitamins.
• For each medication, include the drug name, reason you are taking it, the dosage and instructions, the name of the prescribing physician, and the date it was prescribed.

Preventive Care Schedule

• Dates of your last preventive care exams and routine screenings.
• Include copies of your results if you have them.

Major Health Issues

• List of major illnesses or injuries.
• List of surgeries or hospitalizations.

Family Medical History

• List of diseases that may run in your family such as cancer, diabetes, heart disease, or stroke.

Legal Health-Related Documents

• A copy of advance medical directives including your living will.
• A copy of your medical power of attorney.
