Our website is designed to give you all the information you need to serve Care Assured members and to be reimbursed in a timely and efficient manner.

HealthMarkets Care Assured is a Medicare Advantage Private Fee-For-Service Plan (PFFS), under a contract with the Centers for Medicare and Medicaid Services (CMS). A Medicare Advantage PFFS plan works differently than a Medicare supplement plan. Your doctor or hospital is not required to agree to accept the plan’s terms and conditions, and thus may choose not to treat you, with the exception of emergencies. If your doctor or hospital does not agree to accept our payment terms and conditions, they may choose not to provide health care services to you, except in emergencies. Providers can download the plan’s Terms and Conditions.

Providers can realize many benefits in serving Care Assured members including:

No Contracts — Care Assured does not require you to sign a contract. Any licensed provider who is eligible to receive payment under Original Medicare, agrees to provide covered services to our members, and has reasonable access to the Care Assured Plan Terms and Conditions is considered a “deemed provider.” Our Terms and Conditions are on this website. They can also be accessed by calling Care Assured at 1-877-219-5460 and requesting a copy.
No Precertification and No Referrals — Less paperwork reduces the administrative burden on you and your staff.
Simplified Billing — You can submit one bill directly to Care Assured.
Lower Copayments / Coinsurance — Out-of-pocket cost-sharing to the member is decreased so members will be more likely to schedule needed appointments.
Medicare Reimbursement — Care Assured uses Medicare’s pre-determined payment methodologies.
