Claims Submission

Submission of claims is easy with HealthMarkets Care Assured. For your convenience, we support both paper and electronic submission of claims. For all claims, please be sure to include:

• Medicare Provider Number
• Federal Tax Identification Number
• Member’s ID Number (found on member’s ID card)
• National Provider Identification Number (NPI)

To enable the submission of clean claims – and appropriate and timely payment – please follow the billing instructions listed below:

• Submit claims using the same coding rules as original Medicare.
• Bill all primary and secondary diagnoses.
• Use Medicare CPT codes and defined modifiers.
• Bill diagnosis codes to the highest level of specificity.
• Use CMS-approved HCPCS codes and modifiers.

For electronic claims, use the EDI 837 billing forms.
